Your First Visit
We are so excited to see you for your first dental appointment with us! Upon your first visit to our office, you will be greeted by one of our friendly front desk team members. Our patient services team will be available to assist you should you need any help filling out your forms. Please arrive approximately 10 minutes before your appointment time to complete the necessary forms we require prior to treatment. It is important that you bring with you information concerning your past medical history, dental insurance information, and any medications you may be taking. If you wish to complete these forms prior to your appointment, please see the attachments below.
Financial Policy
We are happy to offer direct insurance billing to most insurance companies as a courtesy for our valued patients! As part of our patient services, we aim to make your visit as smooth and stress-free as possible by assisting you with insurance processing. Please keep in mind that your insurance policy is an agreement between you, your employer, and your insurance company. It is the responsibility of the policyholder to notify our office of any policy changes so that we can continue to support you in maximizing your dental benefits.
Appointment Policy
Broken appointments are a disappointment to everyone. They can interfere with treatment progress and create scheduling challenges for future care. At Village Walk Family Dental, our patient services are designed to accommodate the needs of our valued patients by providing high-quality dentistry, personalized treatment options, and exceptional service.
To ensure this level of care, we schedule each patient in a specially reserved time slot tailored to their individual treatment needs. When an appointment time is agreed upon, we consider it a mutual commitment. We kindly request two business days’ notice if you need to change an appointment. This allows the Village Walk Family Dental team to manage our schedule efficiently and respect the time of our doctors and dental hygienists. We understand that emergencies can arise, and in such cases, appointments will be assessed on an individual basis.
